MedStar Home Health is a full service, home care provider. Our highly skilled team of professionals provides a customized plan of care that is unique to each patient in the comfort of their home. It is our duty to educate the patients and their families throughout the course of care and have someone on call 24/7. We provide the original assessment and all reassessments needed for each patient, throughout the course of care.

Social Service Features

• Referral to community resources.
• Nursing home or institutional placement assistance.
• Providing rehabilitative and supportive case work.
• Assisting the patient and/or caregiver, family and team members with social, emotional, and environmental issues relating to the patient's and/or caregiver's health problems.
